Douglas Gardens Community Mental Health Center is looking for a Critical Time Intervention care coordinator. Previous experience working with the mental health/co-occurring population utilizing outreach services preferred.

Critical Time Intervention is a time-limited evidence-based practice that mobilizes support for vulnerable individuals during periods of transition. It facilitates community integration and continuity of care by ensuring that a person has enduring ties to their community and support systems during these critical periods.

The CTI care coordinator will primarily be working in the community engaging and assisting clients. The average assigned caseload will consist of 25 clients.


  • Bachelor’s degree in social work, psychology or a related discipline from an accredited college or university.
  • Minimum of 2 years of experience providing case management services and/or outreach services to a vulnerable population, preferred.
  • Bilingual in English and Spanish required.
  • Must have reliable transportation, valid Florida Drivers’ license and current automobile insurance coverage.

This is a full time position, Monday to Friday 8:30 a.m. – 5 p.m. Benefits include health insurance, dental, vision, short-term disability, long-term disability, life Insurance, 401k and paid time off.
